Report of Findings
Licensing received a complaint on October 17th, 2019, regarding physical facility and care and supervision.
The allegation included the following details:
- Failing to secure children in high chair
- Inadequate supervision while diapering
- Broken equipment
- Failure to follow facility's sunscreen procedure
All areas of the complaint were fully investigated. Evidence was collected from interviews, records, and observation. Licensing determined the following contraventions:
