Report of Findings
Licensing received a complaint on October 5, 2020 alleging concerns regarding physical facility and hygiene and communicable disease control.

The allegations included the following details:
- unsafe condition and cleanliness of furniture and equipment
- inadequate hygiene practices

All areas of the complaint were fully investigated. Evidence was obtained through interviews, observations.
Licensing determined the following contravention:

Observation: Furniture was observed to be visibly dirty and in poor repair.
