Report of Findings
Licensing received a complaint on August 15, 2022, alleging concerns regarding care and supervision and notification of injury or illness.
The allegations included the following details:

Care and Supervision
- A child was unsupervised while on a field trip to Lynn Canyon Park.

Notification of Injury or Illness
- The Licensee did not immediately inform a parent that their child was unsupervised while on a field trip to Lynn Canyon Park.

All areas of the compliant were fully investigated. Evidence was collected through documents obtained from the facility, interviews, and a review of the field trip location by the Licensing Officer. documentation.
Licensing determined the following contraventions:
- Community Care and Assisted Living Act (CCALA) 7(1)(b)(i) Health, safety and dignity of persons in care.
- Child Care Licensing Regulation (CCLR) 39 (1) Children are supervised at all times.
- Child Care Licensing Regulation (CCLR) 55(1) Parent immediately notified if a child is involved in a reportable incident.
- Child Care Licensing Regulation (CCLR) 55(2) Licensing Officer notified within 24 hours if a child is involved in a reportable incident.

14060 - CCLR s.39(1)A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.
Observation: During this investigation it was determined that a child was unsupervised while on a field trip to Lynn Canyon Park.
Corrective action: Corrective action required.
19020 - CCLR s.55(1)(b)A licensee must immediately notify a parent or emergency contact if, while under the care or supervision of the licensee, the child (b) is involved in, or may have been involved in, a reportable incident described in Schedule H.
Observation: During this investigation it was determined that a parent was not immediately notified when their child was unsupervised during a filed trip to Lynn Canyon Park.
Corrective action: Corrective action required.
19030 - CCLR s.55(2)(a)A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee.
Observation: During this investigation it was determined that the Licensee did not notify the licensing officer within 24 hours that a child was involved in a reportable incident as described in Schedule H as required.
Corrective action: Corrective action required.
50050 - CCALA s.7(1)(b)(i)A licensee must do all of the following: (b) operate the community care facility in a manner that will promote (i) the health, safety and dignity of persons in care.
Observation: During this investigation it was determined that the Licensee did not adequately plan for and assess the location of a field trip and placed the children in care at risk.
