Report of Findings

Licensing received a complaint on June 9, 2022 alleging concerns regarding:
· Records and Reporting
· Staffing
· Care and Supervision
Continuous Supervision required
Behavioral Guidance
Group sizes and Employee to children ratios
· Policies and Procedures
· Physical Facility

The following concerns were raised:

- children's records are not up to date
- staff have expired first aid
- staff are not adequately supervising children
- staff yell at children
- a staff working as additional support was considered to be in ratio
- staff did not follow safe release of child procedures
- lack of facility cleanliness

All areas of the complaint were fully investigated. Evidence was collected through interviews, on-site inspections and/or review of documentation. Licensing determined the following contravention:
