Report of Findings
On November 17, 2020, Licensing received a complaint alleging concerns related to care and supervision. The allegation included the following details:

- Persons in care were involved in a reportable incident.

All areas of the complaint were thoroughly investigated. Evidence was received and reviewed from interviews, and correspondence with the facility.

The following contraventions were determined:
- Residential Care Regulation (RCR) 61 – Self monitoring of community care facility,
- RCR Section 77 (2)(d) – Reportable incidents,
- RCR Section 81 (4)(b)(i) – Care plan needed if more than 30 day stay, and
- RCR Section 82 - Implementation of care plans.
Substantiated Contraventions: Evidence for this summary report is based on a combination of the Licensing Officer’s observations, a review of facility records and information provided by the facility staff at the time of the investigation.

30240 - RCR s.61A licensee must regularly monitor the physical environment of the community care facility, and the care and services provided by it, to ensure that the requirements of the Act and this regulation are being met.
Observation: Licensing determined there were many gaps in documentation that staff were to complete.

34750 - RCR s.81(4)(b)(i)A licensee must ensure that (b) each care plan is reviewed and, if necessary, modified (i) if there is a substantial change in the circumstances of the person in care.
Observation: Licensing determined a care plan was not current.

34780 - RCR s.82A licensee must ensure that the care and supervision of a person in care is consistent with the terms and conditions of the person in care's care plan.
Observation: Licensing determined staff were not following care plans.
