311 | Critical Other (specify below) Observation: Inspection in response to all three July 7, 2011 samples being positive for total coliform - Site 16 -STC; Site 37-STC; Site 17-GTC. Met with operator for inspection of the system. Noted that on UV monitor lights for lamps 5 and 6 were flashing on and off. Operator reports just having replaced one of these bulbs. Corrective Action: Operator determined that cause of flashing lights for 5 and 6 as noted in July 12 inspection was due to faulty ballast and therefore lamps 5 and 6 may not be producing UV. |
313 | Inadequate treatment Observation: Residual chlorine tested at three points in distribution system - trace amounts detected, ~0.04 free chlorine. Corrective Action: Bacteriological sampler collected by EHO from Site 16, 17 and 37. Boil Water Notice pending outcome of July 12 samples. July 14, 2011 - EHO contacted BCCDC for verbal results - Site 17 10 TC; Sit 16, 37 L1/100ml. July 14, 2011 - EHO contacted operator to provide results - learned that chlorine levels in distribution very low again. |
