Spray solution sanitizer concentration insufficient; quats dispenser does not appear to be dispensing at correct concentration. Repair/replace dispenser or manually mix solution per manually mix solution per manufacture's instructions (quats), or 3.33 mL bleach (6%) to 1L water. Quats test strips should be available in order to verify concentration.
116   Improper Construction / Maintenance of Establishment
Ice machine requires cleaning and insides of coolers and freezers.
118   Food Safety Plan/Sanitation Plan
