Observation: The dough mixing area is susceptible to contamination from falling debris, dust, etc due to exposed / unprotected ducting and sheet metal works that have not been maintained.
Corrective Action: Arrange for the installation of a temporary protective barrier and assure the food contact surfaces are maintained in a sanitary manner during food production. See public comments below also:

Observation: Plumbing and sinks not properly sized, installed and maintained. Equipment and floors not properly drained. Cross connections present.
Corrective Action: Please refer to Public Comments below and previous inspection reports which were discussed earlier.

Follow up inspection:

Our previous follow up inspection report of September 15, 2011 noted that condensate drainage lines are still discharging into the 2 compartment sink. The manager was to investigate the origin of this condensate and take the appropriate corrective action to ensure that the drainage condensate is not discharging into the two compartment sink. The target date for completion of this work was extended to the 15th of November , 2011.

Additionally a mop sink was to be installed. This has also been discussed with the manager with regard to the proposed size and the location of the mop sink. It was agreed upon by the manager that a mop sink is needed and will be installed to ensure the filling and the draining of the mop bucket is performed in a sanitary manner. The target completion date for the mop sink installation was also extended to 15th of November, 2011.

At the time of our follow up visit, neither of these items had as yet been addressed. Furthermore, the following additional risks have been noted and discussed with your staff:
The interior surface of the walk in cooler must be cleaned and sanitized to remove the accumulated deposits.
The acoustic ceiling panels are missing over the dough mixing area. It appears some work is anticipated to the mechanical / vent duct systems however in the meantime there is an increased risk of food exposure to dust and debris which may fall from behind the ceiling. This area must be vacuumed (likely with a professional quality - high volume vacuum) before replacing the tiles. Care must be taken to protect all food contact surfaces in the meantime. In this regard your facility has been reclassified to a Moderate (M) hazard rating.

A reinspection is required and will be scheduled. Please advise our office of your intention to correct the above noted deficiencies at your earliest convenience.
