Observation: 1) Cooked pizza slices observed holding between 30C and 40C. Several slices pizza discarded during inspection. Time/temperature monitoring was not evident. 2) Tomato sauces on prep line observed held at room temperature. 3) Frozen cooked chicken wings observed thawing at room temperature.
Corrective Action: 1) Cooked foods must hot-hold above 60C, or refrigerated at 4C. Please provide Panago Food Safety Plan regarding hot-holding pizza. 2) Tomato sauces on prep line must be refrigerated or stored on ice. Please provide Panago Food Safety Plan for pizza sauces. 3) Frozen cooked chicken wings to be thawed under refrigeration, or cooked from frozen state. Please provide Panago Food Safety Plan for chicken wings.
116   Improper Construction / Maintenance of Establishment
Observation: 1) Staff washroom - hot water tap leaking and wall-mounted liquid handsoap dispenser broken. 2) Janitorial sink - hose attached to faucet lying on sink bottom. 3) Many serving trays/dough proofing pans, are cracked and damaged. 4) Door handle to walk-in cooler still broken (outstanding since October 2007).
Corrective Action: 1) Repair leaking tap and broken soap dispenser. 2) Hose attached to janitorial sink faucet should removed when not in use, or hung outside of sink, to prevent possibility of cross-contaminating public water system. 3) Replace any cracked/damaged trays, to provide food contact surfaces that are smooth, impervious and free from cracks and breaks. 4) Repair broken door handle on walk-in cooler.

Observation: 1) Permit to Operate is not posted in location visible to the public. 2) Ceiling light in food preparation area is not covered with shatterproof cover.
Corrective Action: 1) Permit to Operate must be posted in location visible to the public. This is stated on the Permit. 2) Replace missing shatterproof plastic covers on ceiling lights.

- Please provide names of foodsafe certified staff. At least one Foodsafe certified employee must be on site at all times facility is open for business.
