Observation: Deep well was covered/insulated but well cap had been removed and placed on the ground.
Corrective Action: Ensure well is capped, sealed, and protected. Additionally, ensure a sufficient amount of stick-up is available, and the base of the well is mounded as discussed during the on-site inspection.

Observation: There is no recent record of chemical analysis on this source.
Corrective Action: Chemical analysis is required every 5 years as per condition on the Operating Permit. Please Submit a chemical analysis of source water by June 2018.
