306  Critical Chemical contamination of raw water
Observation: Emergency Actions - Operator reported that an error was made that resulted in the injection of a significant amount (approximately 200L) of 12% chlorine being injected into the distribution system without appropriate dilution taking place. The EHO was notified by the operator once the issue was identified and the emergency messaging was sent out by the Town. Due to the delay in recognizing the error, by the time the notification was going out, the risk level was already reducing due to flushing and dilution. Locations with identified high chlorine levels continued to be flushed. Follow up actions - there is no procedure for the disinfection and flushing program in place.
Corrective Action: Emergency Actions - Stop injection of chlorine into the distribution system (conducted prior to EHO involvement), flush water lines at appropriate locations (started prior to EHO involvement but consultation/direction at time of inspection), and issue an advisory to the Town of Smithers to ensure residents were aware of associated risks. Follow Up Actions - 1. Operator is to create an annual disinfection and flushing proedure. This procedure is to be forwarded to the EHO for review and acceptance prior to re-establishment of the program. 2. A debreif of the incident is required with the EHO, operator and assigned Town of Smithers operational management staff.
