Observation: Lid of the cistern of the well water system was not present or not completely secured.
Corrective Action: Water must be protected from potential entry of contaminants. Place and secure a lid on the cistern.
317   Inadequate or improper construction of water works
Observation: Operator showed the two locations in the distribution line of the well water system that are used as fill points for water hauling truck. The construction and design of the fill points were improper which may pose a health hazard in contamination the water supply system. The fill points were located directly on the ground and the outlets were not capped.
Corrective Action: Propose a plan to ensure that water supply system and its access points are protected from potential contamination, damages, or tampering. The fill points should be elevated from the ground and these structures should be protected from damages from its surrounding. This plan is to be submitted to your district EHO by May 10, 2017 for reviewing then a timeline for rectifying issue will be set. Immediately cap the outlets and ensure it is capped at all times when not in use. Outlet is to be sanitized and let water run prior to filling water truck.
318   Inadequate Microbiological Analysis data
Observation: Bacteriological water sampling is conducted intermittently. No sampling was done in the last two months. This is a recurring violation that has been noted in previous inspections.
Corrective Action: As per Permit conditions, bacteriological water sampling is required at least once a month. Submit a water sample immediately and going forward on a monthly basis.
319   Inadequate Chemical Analysis Data
Observation: Chemical water sampling was not conducted in the last 3 years. This is a recurring violation that has been noted in previous inspections.
Corrective Action: As per Permit conditions, chemical water sampling is required every 3 years. (1) collect a raw water sample and submit to an accredited lab for testing a list of chemical parameters immediately. The list has been provided to you. (2) Determine the type of treatment of your system then, collect a treated water sample and test for the chemical parameters that the water is treated for. Provide the test results to your district EHO for reviewing by May 24, 2017.

Observation: Inside of the cistern of the well water system could not be inspected due to confined space. However, stains/debris on the cistern's interior walls was seen from the outside.
Corrective Action: Water holding tanks are to be cleaned, flushed, and disinfected more regularly, at least once a year and when required.
