Follow-Up Inspection Conducted

Observations:
- General sanitation of premise is satisfactory
- Hand washing station in back preparation area supplied with hot and cold running water, liquid soap, and paper towel
- QUATS sanitizer at front service measured at 200 ppm QUATS concentration with test strip
- Preparation sink faucet in the back has been repaired. Faucet is no longer leaking. Hot and cold water are available at the tap
- No signs of pests during time of inspection

Notes:
- No signature due to COVID
- Pending FOODSAFE Level 1 certification for staff on site. Proof of completion to be submitted to the health inspector by June 8, 2022
