Observation (CORRECTED DURING INSPECTION): *Repeat violation*
One bucket containing quaternary ammonium compounds (quats) sanitizer was measured at almost no detectable quats (0 to 100 ppm quats). Bucket contained a small amount of sanitizing solution and was observed to be filled upon inspector arrival. No other buckets/bottles of sanitizer were available at time time of inspection.
Corrective Action(s): Bucket was emptied and refilled at the dispenser and measured 200 ppm quats. Ensure that at all times, sanitizer is maintained at concentrations required for effective sanitizing of food contact surfaces (200 ppm quats). Test sanitizer after refilling to verify the concentration (quats test strips are available on site). Note: Follow manufacturer's directions for dispensing sanitizer. Valve at dispensing unit should be opened fully for dispensing sanitizer. Note: Repeat violations may result in further enforcement action.

Knife stored in space between preparation cooler and preparation table, as noted on previous report.
Corrective Action(s): Knife removed. Ensure that knife is stored in a manner that prevents potential contamination of knife. Above-noted space cannot be effectively cleaned and sanitized.

- Food is not stored directly on the floor
- Personal items stored in designated area away from food
- Designated hand washing station cold water tap has been removed. New tap is yet to be installed. Note: Hot and cold running water available at adjacent preparation sink (unobstructed at time of inspection). Liquid hand soap and paper towels available.
Note: Ensure corporate sanitation plan is being followed.
Correction ORDER issued for lack of designated hand washing station. Follow up inspection to be conducted on May 2, 2019.
