Observation: Hand washing sink in front and back was occupied at time of visit.
Corrective Action(s): Hand washing sink must be available ONLY for hand washing with supply of paper towel and liquid soap at all time.

Observation: -Foods noted beside hand wash sink, in back kitchen beside sink and in coolers/freezers without proper protection.
-Janitorial sink is being used as food perp sink
Corrective Action(s): -All foods to be in food grade containers with proper lid at all time.
-No food prep in mop sink, there are two compartment sink in the back use those

Observation: Back door was left open at time of visit and a lot of clutters in the facility
Corrective Action(s): -keep the back door closed at all time
- keep all items at least 6 " off of the floor to aid cleaning and monitoring for pest activity

Observation: Poor cleaning noted throughout the facility- walls, counters, sinks, under sink, inside coolers and freezer, around cooking equipment, behind and under all equipment, all equipment (exterior)
Corrective Action(s): Clean and sanitize the entire facility.

Close the facility immediately- address all noted issues- when ready call the area inspector to schedule an inspection.

** Noted some dropping but due to unsanitary conditions not able to verify active or past rodent issues. Clean the area and put sticky traps to monitor if any issue noted call pest control company for service.
