302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
Observation (CORRECTED DURING INSPECTION): Multiple pots/pans/utensils were being washed and rinsed on the floor in the large room with the floor drain, while volunteer was standing barefoot in the floor that had pooled brown-ish colored water. Many pots/pans stored directly on this dirty floor. Some of these pots/pans can easily fit in your dishwasher.
Corrective Action(s): All utensils/pots/pans that can fit in your dishwasher MUST be washed/rinsed/sanitized in the dishwasher area where you have the large sink to wash them. This large room with the floor drains will need to be addressed as it does not look like it is being maintained in sanitary condition and your large pots (that don't fit in the dishwasher) should not be washed/sanitized on the floor at any time.
Do the following:
1) Move all smaller dishes/utensils to the dishwashing area
2) wash all big pots off the floor in the large room (with soap and water first). then rinse off and soak in bleach solution for 2 minutes, and then pour out the bleach solution. Let the big pots air dry (do not wipe them dry).

209 - Food not protected from contamination [s. 12(a)]
Observation: Rodent feces observed on several food items in dry storage area, as well as inside the pizza making machine. Please refer to above violation for "premises not free of pests". There are several boxes in the dry storage room that have plates, utensils (knives, forks), and other items that come into contact with food. These boxes do not have lids, and some of them are milk crates.
Corrective Action(s): 1) Ensure all food items are always covered
2) Instead of using boxes with no lids or milk crates, use plastic bins WITH lids for all your utensils, plates, etc in this dry storage room.

Observation: Rodent droppings still observed in several areas:
1)Basement food storage room (on top of cardboard boxes, on top of plastic bins/lids, along the floors/corners, under shelves, along the lower shelf where the extra utensils are being stored (this was pointed out to one of the staff/committee members of the temple)
2)stairs leading down to the basement and the hallway in front of the door by the stairwell
3)Laundry room (floors beside the washing/drying machines), in the corners
4) Inside the pizza making machine (as shown to staff)
Corrective Action(s): Remove all rodent droppings and clean affected areas with bleach water sanitizer (1 tsp bleach per 1 liter of water).

Observation: Several holes in the ceiling of the basement dry storage room:
-along the corner by the utility box
-in the far left corner where the futon mattress was placed on the top shelf
Also large hole around wiring coming from the utility box in the basement room
Corrective Action(s): Seal ALL gaps and holes to prevent pest/rodent entry.

Comments

Facility has gotten more organized since the previous inspection, there has been decluttering in several areas throughout the kitchen and in the basement food storage area.
Pest control technician was on site during inspection. He provided a copy of his report (picture taken). Previously noted affected areas (under the handsinks, in some areas of the dry storage area) have been cleaned since last visit.

The following must be done:
1) Maintain pest control monthly and e-mail your district health inspector the reports each month. (business card provided with e-mail address)
2) Check for signs of recent pest activity (droppings, open food packaging, holes, dead/live rodents) EACH morning and night.
3) Clean all affected areas with droppings with bleach water (as mentioned above).
4) Protect food from contamination by covering all food containers/utensils in containers with tight fitting lids.
