304 - Premises not free of pests [s. 26(a)]
Observation: Rodent droppings observed in the following areas:
> Far right storage closet on Tendon side (closest to kitchen): 1 dropping under the shelf across the door.
> Middle storage closet on Tendon side: droppings and urine marks along the wall leading into the crawl space area, 1 dropping noted at the end of the walk-way on the other side of the wall, and a few observed behind the 2nd door.
> Middle 2-door cabinet across from customer washrooms: droppings between the lower shelf and bottom of the cabinet.
Corrective Action(s): Remove droppings. Sanitize the areas with a strong bleach solution to remove scents. Continue to monitor for new activity.

306 - Food premises not maintained in a sanitary condition [s. 17(1)]
Observation: Cleaning is required in the following areas:
> Walk-in cooler: food debris in the corners of the cooler underneath storage racks.
> Tendon side vent hood: grease marks on the left side panel under the hood.
> Far left storage closet on Tendon side (closest to mall entrance): mould growth observed inside the 2 unused coolers. Significantly moist and warm inside these units.
> Middle storage closet on Tendon side: dry goods (tempura batter mix, flour) observed on top of the pest proof bin lids.
Corrective Action(s): Clean and sanitize as per your sanitation plan. Accumulated food debris and grease act as food sources for rodents.

Remove mould growth from inside the unused coolers. Cooler doors should be propped open for some time to allow the excess moisture to escape. Warm and moist conditions will facilitate mould growth.

Staff informed that owner has plans to remove these 2 coolers. Facility stopped using these coolers approx 2 months ago.

307 - Equipment/utensils/food contact surfaces are not of suitable design/material [s. 16; s. 19]
Observation: Handsink on the Tendon side located between the 4 burner stove top and deep fryer does not have splash guards on either side of the basin. Basin is in close proximity to cooking elements that can be hazardous.
Corrective Action(s): Install splash gaurds on either side of this handwash basin.

Date to be corrected by: 1 month. Call EHO with timeline for completion.

309 - Chemicals, cleansers, & similar agents stored or labeled improperly [s. 27]
Observation: 2 spray bottles in the facility were unlabelled but contained a bleach sanitizer solution: 1 white spray bottle on the Tendon side server station, and 1 white spray bottle on the Kokoro side server station.
Corrective Action(s): ALL spray bottles must be clearly labelled to identify the contents.

311 - Premises not maintained as per approved plans [s. 6(1)(b)]
Observation: Missing tiles on the kitchen floor:
> Kokoro side: 5 half tiles in front of the deef fryers and stove top.
> Tendon side: 5 tiles in front of the deep fryers (some half tiles, 1 whole tile)

The missing tiles are mainly along the gutters for the floor drain on both sides of the kitchen.

Significant water leaking on the otherside of the dishwashing area, leaking into the hallway by the customer washrooms.
Corrective Action(s): Replace missing tiles. Look into the cause of the leak coming from the dishwasher side of the wall.

> Ice cream machine was not in use at time of inspection. Staff informed it is only used on weekends (Friday through Sunday).
> When in use, ensure both the added ice cream mix and storage chamber are 4C or colder.

Temperature controls - Tendon Kohaku side:
> 2 door prep cooler (by ice machine): 0C
> 2 door prep cooler (serving window): 2.9C
> 2 door prep cooler (tempura station): 1.9C
> 2 door prep cooler (by dishwashing): 3C
> 1 door prep cooler: 2C
> Hot holding (rice, miso soup, various sauces): 60C or hotter
> Small batches of room temperature held tempura: ranging from 34C to 45C - all items are time-tracked for 2 hours. Leftovers after 2 hours are discarded.

General:
> Running hot and cold water is available.
> Handwash stations are fully supplied with liquid soap and paper towels. **
> High temperature dishwasher had final rinse temperature of 74.9C at the plate level.
> All spray bottles contained 100-200ppm bleach sanitizer.
> Vent hood filters are in sanitary condition. Last professional cleaning was done in November 2023 - not due again until July 2024.
> Pest control services are by Jun Pest Control every 2 weeks.
> Last report from Feb 27 was reviewed - no activity noted.
> Foodsafe requirements are met. Kitchen manager has valid level 1 that expires on June 14, 2026.
> Health permit posted at main entrance.

Notes:
* At start of inspection, air temperature inside this cooler was 9.7C. Noted that 4 inserts at the insert level were missing, and cold air was escaping into the surroundings. Temperature decreased immediately once inserts were added back.
** Handwash station on the Tendon side in between the stove top and deep fryers is situated in such a way that may be hazardous; the basin is very close to cooking elements with no barrier or splash guard on either side. See violation 307.
