Observation: During time of inspection, the following was observed
1) rodent droppings observed on ledges of unused dining area
2) rodent droppings located in kitchen premises on floor behind gas (i.e, C02) canisters

Corrective Action(s): - Ensure that ALL rodent droppings are removed and that ALL affected areas are cleaned and sanitized.
- Ensure to incorporate a daily checklist for pest monitoring into sanitation plan

305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
Observation: During time of inspection, the following was observed:
1) Door from outside leading into unused dining area, has gaps underneath, this can be a potential entry point for rodents
2) Gap observed underneath door leading into kitchen from North side
3) Outside, large gap observed where foundation of restaurant and concrete meet
Corrective Action(s): - Ensure that ALL entry points are sealed to prevent pest infestation
- Any small holes must be blocked/ sealed with - steel wool, hard wire mesh or pest block spray foam
- Any large holes can be blocked with hard durable materials, i.e., concrete, cement, wooden blocks, that can be maintained in good sanitary condition

Observation: Commercial fume hood still requires cleaning
Heavy grease accumulation apparent on metal slats/ baffles
Corrective Action(s): Ensure that slats are cleaned frequently by staff/ operator, in addition professional cleaning

Follow up inspection conducted for report # MTRG-APMSHY
The following violations have been corrected since last inspection.
- No potentially hazardous food products were found being stored/ displayed at room temperature during time of inspection
- Facility has undergone thorough and deep cleaning, facility being maintained in sanitary condition
- Rodent droppings have been removed from shelving around coffee station, underneath hot holding units, cardboard pop syrup boxes, on floor, behind dry good bins
- Operator provided invoice/ assessment for pest control, Pest Control Operator is now being employed on bi-monthy basis
Notes: CONTINUE TO MONITOR AND CLEAN AND SANITIZE ALL AFFECTED AREAS
ENSURE THAT ALL PEST CONTROL REPORTS ARE LOCATED IN CENTRALIZED LOCATION
- All handwashing stations/sinks adequately equipped with hot/cold running water, liquid soap and paper towels
