Observation (CORRECTED DURING INSPECTION): - Designated handwasing station located at cook line had equipment/utensils stored within basin.
- Designated handwashing station located near back prep area had food stored within basin (bottle, bag of onions). In addition, boxes were obstructing access to handwashing station.
Corrective Action(s): Items removed. Ensure handwashing stations are accessible for hand washing at all times.

Observation (CORRECTED DURING INSPECTION): Staff observed:
- washing hands using water only (no soap)
- washing gloved hands
- touching clothing with gloved hands, and then using the same gloves for cooking/food preparation.
Corrective Action(s): Ensure staff wash hands properly using warm running water and lathering with liquid soap, and then drying with a single service paper towel. Ensure staff wash hands as often as necessary to prevent the contamination of food. Ensure staff change gloves as often as necessary to prevent the contamination of food; discard gloves and wash hands before donning a new pair of gloves.

- Raw and ready-to-eat properly organized.
- No food observed in open cans.
- Dry food product stored in bins with lids. No evidence of contamination observed.
- Bean sprouts stored with ice.
- Equipment and utensils in sanitary condition
- Few rodent droppings observed in hard to reach areas (storage/floor). Ensure areas are cleaned and disinfected.
