General Comments Handsinks equipped with hot and cold running water, liquid soap and paper towel *Paper towel dispenser at one handsink jams easily and therefore staff avoid using it. Please address this, as facility was approved based on having fully equipped handsinks at each work station. Paper towel must be easily accessible to staff Sanitizer solutions must be changed more often in order to maintain 200 ppm Quats concentration 3-compartment sinks must be drained and refilled more often to maintain clean water and correct quats concentration (3-compartment wash method: 1. pre-scrape 2. first sink - detergent and warm water 3. second sink - rinse with warm water 4. third sink - soak in quats sanitizer at 200 ppm 4. air dry) Quats sanitizer at dispenser was 200 ppm; test strips available *expire December 2018 Coolers operating at </= 4C Freezers operating at </= -18C Temperature logs in-use Hotholding operating at >/= 60C Ice water cups for in-use stir spoons at </= 4C No signs of pests observed General sanitation is adequate
