301 - Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
Observation (CORRECTED DURING INSPECTION): Scoops with rice and raw tuna debris were observed stored in stagnant water which was measured at 23 degrees C.
Corrective Action(s): Above-noted scoops and containers were washed and sanitized at time of inspection. It was discussed with the owner that scoops should be stored in ice water at all times (at 4 degrees C or less) OR scoops may be stored in a clean dry container. If scoops are stored in a clean dry container, both the scoops and container must be properly washed and sanitized at least every four hours. The owner stated that he is planning to implement a timer system and staff education to ensure the above-noted conditions are met.

302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
Observation (CORRECTED DURING INSPECTION): 1. Quaternary ammonium compounds (quats) sanitizer was not detectable in the container locted at the front. A small amount of quats solution was present along with wiping cloths.
2. Quats sanitizer was measured at 100 ppm or less in the container at the back. A small amount of quats solution was present along with wiping cloths.
Corrective Action(s): It was discussed with the owner that quats solution should be changed as often as needed to maintain effective sanitizing concentrations (200 ppm quats). Please refer to sanitizer concentrate bottle for manufacturer's information on effective sanitizing concentrations for food contact surfaces (no rinse). Test strips are available on site. It is recommended to fill containers with more sanitizing solution to mitigate the depletion of sanitizer by any organic debris that may be present on wiping cloths. Owner stated that he is planning on implementing a timer system and staff education to ensure that the above-noted requirements are met.

401 - Adequate handwashing stations not available for employees [s. 21(4)]
Observation (CORRECTED DURING INSPECTION): Back designated hand washing station has a sign that states "do not use." It was stated by the owner that water is leaking from the piping located below the hand washing station. The owner also stated that the counter in which the sink is located had separated from the wall (as noted in the previous inspection report), was repaired on June 28, 2018, and had broken off the wall again one week ago.
Corrective Action(s): Ensure that the hand washing station is unobstructed, in good working order, and supplied with hot and cold running water, liquid hand soap, and single use paper towels. Owner removed the sign at the time of inspection to allow use of this hand washing station. Hand washing station is supplied with hot and cold running water, liquid hand soap, and single use paper towels. A catch bucket is located beneath the leaking piping.

209 - Food not protected from contamination [s. 12(a)]
Observation (CORRECTED DURING INSPECTION): 1. Two paper bags of dry food product were observed on the back room floor.
2. Fly papers with flies were observed directly above food preparation areas.
Corrective Action(s): Above-noted food items removed from the floor at the time of inspection. Ensure that all foods are kept off the floor to limit potential contamination and to facilitate cleaning of floors. Bagged dry storage food items should be transferred to covered, foodgrade, pest-proof containers once opened.
Fly papers to be immediately removed. Ensure that these are not located in areas where food is prepared, or where food and/or food equipment is stored.

Observation: Back hand washing station is leaking from piping located beneath sink.
Corrective Action(s): Ensure that hand washing station and piping are not leaking. Date to be corrected by: August 2, 2018.

Follow up to Rpt #MBEK-B25UF5.

- Front under counter cooler was measured at 4 degrees C
- A thermometer accurate to 1 degree C is located within the glass reach in cooler (back area)
- A stainless steel splash guard is in place at the front hand washing station to prevent potentially contaminated water from splashing onto food and/or food contact surfaces
- It was discussed with the owner that daily temperature records will be maintained.
