Observation (CORRECTED DURING INSPECTION): Sweet potato stored at room temperature measured an internal food temperature of 18C. Operator stated the potato was prepared an hour ago.
Corrective Action(s): Move the sweet potato to a functioning cooler immediately. Cold potentially hazardous food must be stored at 4C or less.

Observation (CORRECTED DURING INSPECTION): Bowls of cooked rice stored in one of the rice cooker measured with an internal food temperature of 51C.
Corrective Action(s): Discard all bowls of cooked rice stored in the aforementioned rice cooker.

Observation: Accumulation of food debris observed on mixer arm of the dough mixer.
Corrective Action(s): Clean and sanitize the dough mixer immediately. Educate the staff on proper cleaning and sanitizing procedures. Correct by: today.

Observation (CORRECTED DURING INSPECTION): Food surface sanitizer not available at the time of inspection. Staff did not know the dilution factor for mixing bleach with water.
Corrective Action(s): Operator prepared 200 ppm bleach sanitizer at the time of inspection. Educate the staff on the proper procedure of mixing the food surface sanitizer. Obtain chlorine test strip and make it available on site for staff to use. Clean and sanitize all food contact surfaces immediately.

Observation (CORRECTED DURING INSPECTION): Two trays of raw meat observed to be thawing at room temperature.
Corrective Action(s): Move the raw meat into the walk-in cooler immediately. Frozen food must be thawed under cold running water, inside a functional cooler, or using the microwave. Do not thaw potentially hazardous food at room temperature.

Observation: 1. Leaking cold water faucet observed at the preparation sink near the cook line.
2. Rice cooker unable to hot hold cooked rice above 60C.
Corrective Action(s): 1. Service / fix the leaking cold water faucet. Correct by: Feb 25, 2020.
2. Service / replace the rice cooker to ensure cooked rice can be hot held at above 60C. Do NOT use this rice cooker in the meantime. Correct by: Feb 13, 2020.
