Observation (CORRECTED DURING INSPECTION): Time tracked individual slices of pizza at the front counter did not have time tracking documents for review. Pizza were stored under a hot holding light which was less than 60C.
Corrective Action(s): Operator discarded the aforementioned pizza. Pizza must be time tracked according to the Food Safety Plan submitted with documents ready for review.

Observation (CORRECTED DURING INSPECTION): Two quaternary ammonium spray bottles filled with unknown chemical inside. Test strips turned blue in color.
Corrective Action(s): Discard the content from the aforementioned bottles and refill with 200 ppm quaternary ammonium sanitizer from the dispenser. Clean and sanitize all food contact surfaces around pizza making area.
