Observation: During time of inspection, the following observations were made:
1. Handwashing in front service areas missing soap
2. Handwasing station at cook line obstructed with sheet pan
Corrective Action(s): 1. ALL handwashing stations must be equipped with an adequate supply of hot/cold running water, liquid soap and paper towels at ALL times.
2. All handwashing stations must be unobstructed and accessible at ALL times for frequent and proper handwashing

Observation: During time of inspection, evidence of rodents detected.
- Droppings located under shelving by walk-in cooler
- Droppings located on lowest shelving storing dry goods container
- No foods appear to be affected during time of inspection
Corrective Action(s): - Operator and staff MUST move shelving from wall and assess
- ALL droppings MUST be removed and areas cleaned and sanitized
- ALL entry points must be plugged/ sealed
- Follow Pest Control Operator recommendations
- Continue to ensure ALL foods are kept protected from contamination (i.e., all opened dry goods must be stored in containers)

4. Facility requires deep clean
5. Facility requires de-cluttering of kitchen items/ containers

2. Ensure that all containers are cleaned, wiped down, sanitized and maintained on regular basis
3 -5. Ensure thorough deep clean of facility done and all unnecessary items removed, facility must be maintained in sanitary condition at all times
