Observation (CORRECTED DURING INSPECTION): 1. Inserts of perishable food were stored above the insert line in the prep cooler.
2. Box of thawed fish was stored in the two-compartment sink in the back area for 2 hours. Temperature measured approximately 13-15°C
Corrective Action(s): 1. Store inserts on insert line. Keep extra inserts in the bottom compartment of the prep cooler.
2. Move fish to cooler capable of maintaining 4°C or less.

Observation (CORRECTED DURING INSPECTION): Ice cream scoop was stored in a container of room temperature water. No dipper well available.
Corrective Action(s): Maintain in-use ice cream scoops using one of the following methods:
- Dry in a clean container. Clean and sanitize at least every 4 hours.
- In ice water. Clean and sanitiize at least every 4 hours.
- Clean and sanitize or swap to a new scoop after each use.

Observation (CORRECTED DURING INSPECTION): Bleach sanitizer in kitchen measured 0 ppm.
Corrective Action(s): Prepare bleach sanitizer by mixing 1/2 teaspoon (1/2 cap) of bleach per 1L water. Ensure this is available while kitchen is in operation.

Observation (CORRECTED DURING INSPECTION): Hand washing station in the kitchen did not have paper towels. A staff member indicated a cloth towel was used to dry their hands.
Corrective Action(s): Supply hand washing station with paper towels. Use only paper towels for drying hands.

Observation: Single-use paper cups were being used as food scoops in the bulk food bins.
Corrective Action(s): Use durable scoops with handles to avoid contamination to food. Store scoops so the handle is not touching the food or store outside of the bin in a clean container. Ensure scoops are cleaned and sanitized regularly. Correct as soon as possible.

Observation (CORRECTED DURING INSPECTION): Box of fish was being thawed at room temperature in the two compartment sink in the back area.
Corrective Action(s): Thaw fish and all other frozen perishable food using one of the following methods:
1. In a cooler at 4°C or less
2. Under cold running water
3. In a container of cold water where the water is replaced every 30 minutes

Observation: Rat droppings present on floors along the perimeter of the premises as well as on shelving in the dry storage room. Fly activity observed in the back area.
Corrective Action(s): Clean up all droppings and sanitize affected surfaces. Seal gaps and holes if observed. Reduce food sources and moisture. Clean and sanitize drains to reduce fly activity.
