Introduction

An unscheduled routine inspection was conducted to assess compliance with the Community Care & Assisted Living Act (CCALA), the Child Care Licensing Regulation (CCLR) and the relevant Director of Licensing Standards of Practice (DOLSP). Evidence for this report was based on the Licensing Officer’s observations, review of the facility records and information provided by the facility staff at the time of inspection.Scheduled time for the facility manager to meet with Licensing to review the report due to some changes within the facility requiring amendments.

As part of the routine inspection a Facility Risk Assessment Tool is completed and a copy is provided. The Risk Assessment includes non-compliance identified during the routine inspection and a 3 year “historical” review of the facility’s compliance and operation.

Active and Ongoing Supervision
Fraser Health Licensing is responding to a number of incidents that have involved children going missing or being unsupervised. These types of events can have serious consequences and requires all care providers to take steps to ensure they actively supervise children at all times. Active Supervision, policies and procedures, and design/arrangement of children’s environments, can prevent or reduce the likelihood of accidents and serious injuries.

· Remember: In the event that a serious incident does occur you must report to Licensing within 24 hours. Contact the licensing officer assigned to your facility or phone Fraser Health central line at 604-587-3936 and ask to speak to the Duty Licensing Officer.

Observed Violations
PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 11090 - CCLR s.15(1)(b) - A licensee must supply equipment, furniture and supplies that are (b) located so as not to block or hamper an exit in the case of fire or other emergency.
Observation: Observed equipment / furnishings placed in front of the 2nd exit within the portable therefore restricting clear access to the outdoors. ( It is noted that the portable was not in use during todays' inspection)
Corrective Action(s): please ensure emergency exits remain clear at all times for safe evacuation.

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 11200 - CCLR s.17 - A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation: Cleaners stored under the portable sink and cords hanging down from a microwave presenting risk of injury , the microwave easily slid - did not appear secure
Corrective Action(s): please ensure children do not have access to any item or object which may pose a risk to their health and or safety.It is noted that the portable was not in use during today's inspection.

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 11230 - CCLR s.47 - A licensee must ensure that any heated water accessible to children is heated to no more than 49° Celsius.
Observation: Hot water within the poratble exceeds 49 degrees celsius
Corrective Action(s): Please ensure hot water is reduced to a temperture of 49 or below in order to avoid risk of scalding.

HYGIENE AND COMMUNICABLE DISEASE: 15030 - CCLR s.46(1) - A licensee must establish a program to instruct children in, and to practise the rules of, health and hygiene.
Observation: Children selected snack from their lunch kits while outside , staff observed assisting a child to open a snack. The child was eating prior to sitting at the picnic table , no hand washing took place. Children were asked to wash hands when returning to the classroom however handwashing did not occur staff did not follow through.
Corrective Action(s): please ensure staff model and instruct children in healthy hygiene practices in order to establish a program which promotes children's health and healthy habits.

NUTRITION AND FOOD SERVICES: 17100 - CCLR s.48(4)(c) - A licensee must ensure that children are not (c) left unsupervised while consuming food or drink.
Observation: Two children observed eating at a picnic table while staff provided supervision to children within the playground. Staff were not observed to be in close proximity to the children at all times while eating often walking further away to assist/ remind children to remain within the designated play zones.
Corrective Action(s): Please ensure supervision is ongoing at all times including while children are eating to avoid possible risk of choking

RECORDS AND REPORTING: 19160 - CCLR s.57(2)(i) - A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.
Observation: Of the 7 files audited 2 were found to be missing photos of children.
Corrective Action(s): Please ensure photos or digital images are available in order to quickly identify a child should an emergency occur. Example Missing / Wandering

Comments

At arrival to the facility Licensing observed 20 children with 2 staff outdoors, the staff were observed to be actively supervision however as many children 50 + were also playing on the school ground the area was very active. Staff were observed redirecting children identified to not be attending the school-age program that were playing together.

Staff positioned themselves in the middle of the paved area and were seen conducting head counts and screening the area.

Discussed the practice of transferring children into and out of care from outdoors therefore potentially increasing risk of missing / wandering
Physical Space Requirements
Licence recently amended to remove Preschool which previously operated within a portable, Multi- Purpose Room continues to support a capacity of 21 children. Discussed ensuring this space is set up in advance of children's arrival with age appropriate supplies and equipment and meets for the space requirements of 3.7 m2 useable play space per child
There are no hooks available for children's jackets or system to store their belongings instead jackets were on the floor both inside and outside.
Observed. Stacked chairs , stored piano and large desk area is not used by the School-age program. Please provide in your written response your plan to ensure the area meets for the total capacity of 21.
Licence is issued for 39 School-age with portable used when over 21 as stated by staff likely 1 staff for 3-4 children. An inspection of the portable occurred.

Discussed Washroom Supervision Plan at the review of the report - specifically focused on ensuring the area is clean prior to children's use. Staff stated they check the area in the morning before children arrive however not at the end of the day

Due to electronic failure signatures could not be applied, paper copy provided and signed by both the facility manager and the Licensing Staff.
