Observation: Paper towel in dispenser is noted in staff washroom; however, it's impossible to get paper towel easily from the dispenser.
Corrective Action(s): Ensure paper towel is placed in proper dispenser. Replace with a new dispenser or use a different type of dispenser.

Observation: Floor drain in broiler room is noted with pooling of water, and no drain cover.
Corrective Action(s): Part of dealing with an active cockroach issue in facility is to keep facility clean to help with elimination.

Observation: Some cooking equipment still requires cleaning.

Comments

The following items have been observed:
-->Sanitizer buckets are noted in food preparation areas.
-->Liquid handsoap and paper towel in dispensers are noted at all handwashing stations.
-->All food products observed storing above ground.
-->Apna Pest Control has visited the facility twice (April 1 and April 3) to tackle cockroach issue in facility. No live cockroaches noted in facility at time of visit. One dead cockroach is noted by back door (probably an oversight).
-->Areas that required cleaning are mostly done.
-->Foil and cardboard removed.

Closure Order rescinded. The facility is now given approval to re-open.
