Observation (CORRECTED DURING INSPECTION): Quat sanitizer measured 0 at 3rd sink. Dispenser was out of sanitizer and spray bottles were empty.
Corrective Action(s): Re-fill dispenser at the 3 comp sink and also all spray bottles.

Observation: One of the prep line cooler has loose door.

Hand wash stations provided with liquid soap and paper towels except for one.
Buckets where wiping cloths are stored have 100 ppm chlorine sanitizer.
Walk-in cooler and prep line coolers registered at or below 4 deg C.
Walk-in freezer and line freezers registered -18 deg C or below.
Hot holding for chicken and beef patties was above 60 deg C.
Temperature log was available and up to date.
Dishwasher had >50 ppm chlorine residual on plate surface.
Staff washroom was in satisfactory condition.
Washrooms were in satisfactory condition.
