Observation (CORRECTED DURING INSPECTION): No paper towel in paper towel dispenser at three-compartment sink used for staff to wash hands.
Corrective Action(s): Paper towel was replaced. Ensure a supply of soap and paper towel is maintained at hand sinks so employees can wash their hands.

Observation: Pools of water observed in uneven floor in back storage area by washing machine.
Corrective Action(s): Clean and remove any pooling water in the premises to prevent the attraction of pests.

Observation: Cardboard boxes being used on floor to catch meat debris from meat cutting. Operator states they are discarded at the end of the day.
Corrective Action(s): Cardboard boxes are not durable, or easy to clean. Soiled cardboard boxes can attract pests, and cause a slipping hazard. Remove card board boxes to make the floors smooth and easy to clean.
