Introduction

This is a follow-up inspection completed with the Manager to the routine inspection report # CRAU - C86NXT (completed on October 25, 2021).

Contraventions

Current Inspection - Items reviewed comply with the Act, regulations & standards of practice except for those noted on supplementary pages.

Observed Violations
LICENSING: 30240 - RCR s.61 - A licensee must regularly monitor the physical environment of the community care facility, and the care and services provided by it, to ensure that the requirements of the Act and this regulation are being met.
Observation: A review of the care planning system and persons in care charts indicated that on the Monthly Weight, Blood Pressure and Pulse document that for 1 particular month there is no staff initial. Specifics were provided during the inspection.
Corrective Action(s): Please ensure the care and services provided by the facility are regularly monitored to ensure that the requirements of the Community Care and Assisted Living Act & Residential Care Regulation are being met.

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31100 - RCR s.17 - A licensee must ensure that water accessible to a person in care, from any source, is not heated to more than 49° Celsius.
Observation: A random review of the hot water temperature at person(s) in care ensuite washroom sinks indicated that at a ensuite washroom sink the reading was 50.2 degrees Celsius and slowly increasing. The Manager stated Facilities Management did address this after the Licensing Officer completed the routine inspection in October 2021.
Corrective Action(s): Please ensure the water accessible to a person in care, from any source, is not heated to more than 49 degrees Celsius.

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31290 - RCR s.22(1)(b) - A licensee must ensure that all rooms and common areas are (b) maintained in a good state of repair.
Observation: The items noted in the routine inspection report still need to be completed/addressed.

The Manager stated the kitchen servery countertops are planned to be replaced. Please review the scope of work for this and please submit a health and safety plan if the day to day operation/routines for the persons in care will be disrupted.

The Manager stated the handrails will be proposed to be removed. The Licensing Officer discussed building code requirements for example and that the Licensing Officer will review this internally as well and get back to the Manager.
Corrective Action(s): Please ensure all rooms and common areas are maintained in a good state of repair.
Date to be Corrected: May 31, 2022.

RECORDS AND REPORTING: 39010 - RCR s.49(2) - A licensee must record the height and weight of each person in care on admission.
Observation: A review of the care planning systems and persons in care charts indicated for a person in care that their weight and height upon admission was not documented.
Corrective Action(s): Please ensure the height and weight of each person in care is recorded upon admission.

Comments

Recreation care planning:
- The Licensing Officer met with the Recreation Therapist and the documentation for recreation care planning was reviewed. It appears the intent of the Residential Care Regulation is being met.
Identification when persons in care go on outings:
- Business cards, ID (Identification) bands and bracelets have been implemented.
Wound care:
- There is a wound care binder in-place. If a person in care has wound care as a "focus" for care, then it is incorporated into the care plan with strategies to address this.
Consent to admission:
- The Social Workers for the facility have addressed this to ensure they are in-place for all the persons in care.
Second floor patio:
- The Manager confirmed that persons in care are supervised if out on the patio and/or assessed if they are capable/safe to be on the patio.
Access to outdoor areas on the first floor:
- Discussions are taking place around installation of keypads. In the interim, zipties are placed on the doors accessible to the outdoors and the Charge Nurse checks on the doors during their rounds.
Medication:
- The Manager confirmed that education around documentation for medications has been provided to the care staff.
- The Manager stated the Residential Care Coordinators are following-up on a quarterly basis to ensure the documentation is thoroughly documented (the frequency of checks is increased or decreased depending on the number of issues arising).
Due to the Covid-19 pandemic, the findings were reviewed and discussed with the Manager at the time of the inspection. The corrective dates to address each contravention are noted and a comprehensive written response to this report is also required in-writing by April 8, 2022 (Regarding the hot water temperature, please provide a written response by March 30, 2022 as to how this has been addressed given the temperature was over 49 degrees Celsius and slowly increasing). This inspection report was written off-site and then emailed on March 29, 2022 to the Manager for review and to finalize the report once they were in agreement to the wording. As a result of the pandemic, signature for the Manager is not included. If there are further questions related to this inspection report, please contact your Licensing Officer.
