Met with owner/operator Mr. Gulah and he submitted the following:
Application form and copy of legal documents
Permit fee ($100300 issued cheque #009) from August 2018 to March 31 2019.
Food safety plan and sanitation plan
Food supplier is Sysco and Hawak food
Pest control Ted Terminator
Copy of the F. S. certificate.
Food menu
Trans fats requirements introduced and owner will keep all documentation available onsite.
Floor plan submitted and the following additions have been done:
1 Add prep sink
2. mop sink installed
Additional sink in the dishwashing area, no space to be installed
One of the sinks will be supplied with liquid soap and disposable paper towels.
For now no additional freezers will be needed. Food delivery will be done 2-3 X week.
The following items still need your attention:
1. garbage area need to be organized and pest control provided around garbage bins.
2. Back flow preventer need to be installed in your mop sink to prevent cross contamination of the water supply.
Follow up inspection will be conducted in October 2018.
