Observation (CORRECTED DURING INSPECTION): Found multiple containers of rice ( 45C ) and chicken ( 28 C) in the prep cooler cooling.
Corrective Action(s): Must cool foods in the following manner: cool from 60 to 20 C at room temperature in 2 hrs, then place in a well working cooler to cool to 4 C in 4 hours. Cool in small portions and single layers (chicken), stir regularly where applicable.

Observation (CORRECTED DURING INSPECTION): Found potentially hazardous foods with an internal temperature of 7 to 14 C in the top portion of the sushi bar and both the top and bottom portion of the prep cooler.
Corrective Action(s): Discard all potentially hazardous foods in the 3 coolers.

Observation (CORRECTED DURING INSPECTION): 1. Found dishwasher to have 0 ppm chlorine, no tablets in the machine, none on site. Staff unaware of when tablets ran out.
2. Found wiping rags either on the counter or in 0 ppm chlorine.
Corrective Action(s): 1. Must ensure dishwasher has a final rinse of 50 ppm at all times. Purchase new tablets and ensure machine is capable of producing 50 ppm chlorine in the final rinse.
2. Re-wash all pots, pans, inserts, dishes and utensils after dishwasher has been repaired.
3. Change solution in sanitizing containers every 4 hours or sooner if soiled.
4. Ensure containers have 200 ppm chlorine.
5. Keep rags in the sanitizer.

Observation: Found food and grease build up on floors, walls, equipment (inside and out) and heavy build up of dust on the ceiling tiles and vents.
Corrective Action(s): A deep and thorough cleaning of the entire premises under all equipment, on all shelves, inside and outside of equipment, all walls, and ceiling is to be done.

Observation: Found cardboard, corrugated plastic and plastic wrap on the floor, shelving, counters and walls.
Corrective Action(s): Remove all of the above materials from surfaces in the kitchen as they are not easily cleanable. Wash and sanitize the surfaces as needed and repair damaged walls appropriately.
CORRECT: 1 week for corrugated plastic, today for cardboard and plastic wrap.

Observation: 1. Dishwasher did not have any sanitizer, none on site.
2. Found prep coolers not able to maintain 4 C at the busiest time.
Corrective Action(s): 1. Ensure dishwasher has sanitizer in the machine at all times.
2. Have prep coolers serviced to ensure they are able to maintain 4 C at all times. Do not place hot items into the coolers to help keep other foods cool.
