Observation: Rodent dropping noted in several areas also house flies
Corrective Action(s): - Submit an assessment from professional pest control identifying all point of entries to address
-Insall a light traps ( not close to the food prep)

Observation: -A lot of boxes and buckets noted on floor in front and kitchen
-Back door was left open at time of visit

Corrective Action(s): - All boxes and buckets /other stuff to be on shelving with at least 6" clearance to aid cleaning and monitoring for pests
- Keep the back door close or install a proper screen to exclude flies and rodents entering the facility

Comments

-Good effort noted however there were still outstanding issues (foods and take out containers/ clean utensils are now in closed bins) , shelving units are changed in walk in cooler...) :

- Paper towel dispenser - on order- in the meantime provide a roll near hand washing sink in front

** Rusted shelving units in freezer need sanding /painting or replace them to aid cleaning and provide sanitary surface for food storage
** Rusted shelving in food perp/kitchen- change or repair
** De-cluter the facility to aid cleaning and monitoring for pests

** Ensure to increase frequency and intensity of cleaning ( use bleach solution for floors). Sanitize all surface before any food prep.

Had a phone call conversation with operation manager and discuss the above- 3 months is agreed- please send me a progressive report ( stage every month ) also submit a copy of pest control assessment as received.
