The operator could not say where they were made and did not have any receipts, therefore they are deemed to be from an unapproved source. Corrective Action(s): Removed during inspection. Any food stored on site is assumed to be for sale. Do not store any homemade food items on site. Violation Score: 3

-Bags of dried anchovies were not labelled. Ensure that they are properly labeled with the name of the product, store name, weight and contact information. All coolers were < 4 C. All freezers were < -18 C. No evidence of pests. General sanitation was satisfactory.
