Observation: Mouse activity has increased and cockroach infestation has developed without attention to maintain premise sanitation.
Corrective Action(s): Premise sanitation is to be re-established throughout premise. Upon re-opening premise sanitation is to be maintained on a daily basis.
After thorough premise cleaning has been conducted, pest control services are to audit location to implement appropriate pest control measures including pest proofing, pesticide application and trapping.
All pest control measures identified on service reports are to be implemented

Observation: Gaps around front base of pop display cooler allow for entry of pests into kitchen.
Overstocking in bottled drink closet and crates on floor to do not allow for effective cleaning or pest monitoring.
Corrective Action(s): Pest proof base of pop cooler to exclude rodent entry.
Reduce stock and remove crates from floor to allow for sanitary maintenance.

Observation: Premise sanitation is poor.
Mouse droppings and dried urine observed in the following locations: around handsink and on adjacnet food preparation counter, behind dough roller, under dough roller, under / behind dry storage shelving units, under counter top pop dispenser, on floor under point of sale and around gas cylinders, on floor under pizza oven, on floor around pizza display, on waste bin storage cabinet, on floor under dishwashing sinks, on floor of pop bottle closet, and on floor in front of pop display cooler.
Live and dead cockroaches observed inside pop syrup cabinet, on shelving above prep cooler, on prep cooler, on clean dishware storage shelving above dishwashing sinks and on floor under dishwashing sinks.
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.
Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine 1 tsp bleach / 1 liter water) after cleaning.
