Observation (CORRECTED DURING INSPECTION): No chlorine sanitizer was detected in the rinse residual for the low temperature dishwasher.
Corrective Action(s): A 100 ppm chlorine sanitizer solution was prepared at the three compartment sink. Chlorine test strips available on site.
1. All food equipment/utensils to be re-sanitized as a precaution.
2. All food equipment/utensils that are washed through the dishwasher are to be sanitized manually in the interim until the dishwasher is capable of achieving sanitizing requirements.
3. Switch to single use disposable dishware/utensils only (including for dine-in) until the dishwasher is capable of achieving sanitizing requirements.

Observation: Mouse droppings observed in the following areas:
- Along the floor-to-wall junction and baseboard in the room located near the dishwashing area.
- In the lower cabinet on the opposite side of the wall to the above-noted room. Note: All food is stored in tightly sealed plastic buckets.
- At the front desk where the point of sale is located (all lower storage compartments affected).
Corrective Action(s): Ensure all of the above-noted areas are thoroughly cleaned to remove all droppings and properly disinfected (e.g. with bleach). A stack of take-out paper bags was discarded due to contamination. Date to be corrected by: Today.

Observation: Gaps/holes observed in the following areas:
- Small hole observed inside lower cabinet where dry goods are stored in buckets.
- Hole in the wall (left side) observed in the room next to the dishwashing area.
Corrective Action(s): Ensure above-noted areas are properly sealed to prevent entry of pests at these points. Note: Additional holes/gaps may be present. Monitor and locate/seal as necessary. Date to be corrected by: March 29, 2024.
