Observation: 1.No sanitizer solution available at back kitchen area.
2. interior of ice machine - mildew and brown scum build up in unit
Corrective Action(s): 1.Ensure at least 100 ppm chlorine solution is available with clean wiping cloth to sanitize food contact surfaces.
2. Interior of ice machine must be maintained in sanitary condition at all times. Routinely have the unit cleaned and sanitized to prevent mildew growth. Dump ice and clean and sanitize unit.

Observation: 1.Some mouse droppings noted in corners of back kitchen area and lower shelf of sushi bar.
2. Back door left open during inspection - cardboard barrier built to attempt to prevent pest entry.
Corrective Action(s): Ensure all droppings are cleaned up and mouse activity is monitored and reported to pest control. Pest control devices noted through out facility. Pests appear to be controlled at this time.
Ensure back door remains closed at all times during the cold months when pest activity is on the rise. Keep back door closed.

Observation: Required cleaning:
1. Interior of coolers have food debris and spillage
2. Walls of back kitchen and front sushi bar are covered with food debris
3. Under cooking equipment - heavy accumulation of spilt food, broken dishes, grease
4. Grease accumulation on hood fan - Next service sticker noted: July 2017 (EXPIRED)
5. Lower shelving through out back kitchen- food debris
6. Hard to reach areas under coolers, dish-pit area

Observation: 2 comp sink is pipes are plugged - not draining.
Corrective Action(s): Ensure all sinks are draining waste water properly. De-clog sinks. Operator has called for plumber for service today. In meantime pre-rinse of dishware being done in single compartment sink.
