Introduction

An unscheduled routine inspection was conducted to assess compliance with the Community Care and Assisted Living Act (CCALA), the Child Care Licensing Regulations (CCLR) and the relevant Director of Licensing Standards of Practice (DLSP). All care categories were inspected for compliance.

Observed Violations
PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 11010 - CCLR s.13(1) - A licensee must ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees.
Observation: During the inspection Licensing observed that only one bottle of bleach and water was being used for cleaning the tables and the diaper mat. To reduce the potential for cross contamination two bottles of bleach and water were recommended.
Corrective Action(s): Please ensure a healthy and safe environment is provided at all times.

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 11020 - CCLR s.13(2) - A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation: Licensing observed that several pieces of the plastic tile flooring had puncture marks. This could be difficult to clean and potentially harbour bacteria.
Corrective Action(s): Please ensure the facility and equipment are in good repair.

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 11200 - CCLR s.17 - A licensee must ensure that children do not have access to any object or substance that may be hazardous to the health or safety of a child.
Observation: Licensing observed Lysol wipes and Listerene on the counter in the washroom. The label read,"keep out of reach of children". This was removed during the inspection.
Corrective Action(s): Please ensure children do not have access to any objects or substances that are hazardous to their health and safety.

STAFFING: 12050 - CCLR s.19(1)(b) - A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (b) character references in respect of the person.
Observation: During the inspection one of the employees was in the school playground with 4 children for one hour. The employee does not have character references

STAFFING: 12060 - CCLR s.19(1)(c) - A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (c) a record of the person's work history.
Observation: A record of the person's work history was not obtained for one employee. During the inspection one of the employees was in the school playground with 4 children for one hour.

STAFFING: 12090 - CCLR s.19(1)(f) - A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: (f) evidence that the person has complied with the Province's immunization and tuberculosis control programs.
Observation: Immunization status was not obtained for one employee. During the inspection one of the employees was in the school playground with 4 children for one hour. The employee does not have a record of immunization status.

STAFFING: 12190 - CCLR s.22(2)(b) - A licensee must ensure that each employee (b) practices implementing the fire drill system at least once each month.
Observation: Licensing reviewed the record of fire drills. No record was observed for the months of September and December. In discussion with the Licensee she stated that the fire drill was not practiced in September and December. This is a repeat contravention from inspection #CLAN-BYJSQ8.
Corrective Action(s): Please ensure the fire drill is practiced once a month and that this is recorded.

POLICIES AND PROCEDURES: 13060 - CCLR s.56(1)(d) - A licensee must keep current records of each of the following: (d) written policies and procedures that are intended to guide employees in the care and supervision of children.
Observation: During the inspection Licensing reviewed the policies and procedures. A screen use policy was not observed. This is a repeat contravention from inspection #CLAN-BH6NA5 and #CLAN-B87URJ.
Corrective Action(s): Please ensure a screen use policy is created and available for review.

RECORDS AND REPORTING: 19070 - CCLR s.57(2)(a) - A licensee must keep, for each child, a record showing the following information: (a) name, sex, date of birth, medical insurance plan number and immunization status.
Observation: Three childrens records of seven were reviewed. One child was missing a medical insurance plan number.
Corrective Action(s): Please ensure children's records are complete.

RECORDS AND REPORTING: 19090 - CCLR s.57(2)(c) - A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure.
Observation: Licensing observed that daily attendance was not completed for many children for the last three days. The seriousness of this was communicated to the Licensee and she corrected this during the inspection. This is a repeat contravention from inspections #CLAN-B87URJ, #CLAN-BYJS8Q and CLAN-BSNP3V
Corrective Action(s): Please ensure the attendance records are completed on a daily basis.

RECORDS AND REPORTING: 19100 - CCLR s.57(2)(d) - A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation: Three children's records were reviewed. Two children's records were missing a Doctor's name or phone number. In the absence of a Doctor licensing recommended recording the name and phone number of a medical clinic. This is a repeat contravention from inspections #CLAN-BH6NA5, #CLAN-B87URJ and #CLAN-BYJS8Q

RECORDS AND REPORTING: 19160 - CCLR s.57(2)(i) - A licensee must keep, for each child, a record showing the following information: (i) a photograph or digital image of the child, and other information that can be used to readily identify the child in an emergency.
Observation: Three children's records were reviewed. One child's record was missing a photo. This is a repeat contravention from inspections #CLAN-BH6NA5, #CLAN-B87URJ and #CLAN-BYJS8Q

Comments

At the inspection dated March 8, 2022 there were 3 children present in the facility with the Licensee and 4 children were on the school playground with an employee. At this inspection the above noted contraventions were identified. Please correct the above noted contraventions by March 23, 2022 unless otherwise specified in the report. As part of this Routine Inspection a Facility Risk Assessment Tool is completed and a copy was sent to the Licensee.

The following education/recommendations were provided:

-The reportable incident reporting procedure.
-The current COVID-19 protocols that are in place

The inspection findings were reviewed with the Licensee at the inspection and reviewed with a representative for the Licensee by phone. Due to the COVID-19 pandemic the inspection report and the risk assessment were completed at the Burnaby FHA office. A copy of these documents were sent to the Licensee.
