Follow-up inspection conducted. Significant improvements have been made to the sanitary condition of the facility. The following observations were made:
New tiling has been placed throughout the facility. Equipment has been moved out of the walls and walls have been thoroughly cleaned and degreased
A new stainless steel table has been placed at the back as a "prep table"
A new stainless steel dry storage shelf has been obtained at back-of-house, with wheels
Walls behind ice machine have been throughly scrubbed and is in improved sanitary condition
New overcounter shelving have been added over the prep table
Nails for holding kitchen implements at front-of-house have been removed and replaced with a new rack with stainless steel hooks
Restroom tiles have been completely replaced. New toilet has been obtained and installed
Prep cooler has been repaired and is now running at 2C - excellent
Glasswasher releases 25 ppm iodophor residual at final rinse cycle. Test strips available

Thank you for addressing the concerns raised in the previous inspection. The following will still need to be completed:
Area behind the cook line will need further cleaning
Unused dishes underneath all the chafing dishes will need to be removed. These dishes still have old rodent droppings on them
Old rodent droppings over dish-pit area need to be removed
Existing tiling over the chafing dish area, dishpit area will need some more cleaning. The hole underneath the dishpit will need to be filled
Additional paint work required next to the door of the walk-in cooler. Areas above the tiling of the prep table at back-of-house need to be repainted

Operator has plans to replace the table across from the glasswasher with a quartz countertop (ETA: 1 month)
