Observation (CORRECTED DURING INSPECTION): Cold potentially hazardous foods stored in the display case cooler were note kept less than or equal to 4C. 5 wraps made from previous days had an internal temperature of 10C. Food products were discarded. Staff member claim that the unit was operating at a temperature of 4C or below when the facility first opened.
Corrective Action(s): Continue to monitor the cooler unit and repair or adjust temperature setting if needed. All cooler units must be able to keep food less than or equal to 4C at all times.

Observation: Mold and mildew noted on the drip tray for the condenser panel within the ice machine.

Observation: Cockroaches noted on the sticky traps placed around mop sink and ice machine within the extra storage room.
Corrective Action(s): Operator has an on going contract with pest control company. Ensure to the pest control company is aware of the problem and necessary remediation actions are taken. All equipment, take-out containers, and foods, especially the wheat grass, needs to be properly protected.

Routine inspection conducted and the following were noted:
Both under counter cooler in the retail area were less than 4C.
Yogurt product was less than 4C.
Stand up cooler in excess storage area were less than 4C.
Freezer units in the excess storage were at -18C.
3 - compartment sink method setup and in use. Sanitizing sink was filled with a 200ppm quats solution.
