Licensing received a complaint on July 21, 2023, regarding Physical Facility, Equipment and Furnishings; Licensing; and Policies and Procedures. The following details were provided: - Unsafe or unclean environment - Facility gate is left open and accessible to children - Children playing on unsafe equipment - Licensee limiting the indoor space that children are accessing - Facility doesn’t have sufficient emergency supplies All areas of the complaint were fully investigated. Evidence was obtained through interviews and observations.
