*Follow-up to inspection conducted on September 25, 2018 (Report #MLOO-B4XQK6) - Ice machine has been cleaned - Large hole by the back door has been sealed. Operator will address the gap underneath the back door as well. - Low temperature dishwasher is functioning correctly - 50 ppm chlorine sanitizer dispensed *Continue to clean regularly in all areas to prevent accumulation of food and debris. This includes floors, walls, handles, food contact surfaces, equipment surfaces. Ensure hard to reach areas are also cleaned (underneath and behind equipment). *Remove all items not required for the operation of the business from the premises *Keep all items required for the operation of the business at least 6 inches off the floor to allow for cleaning *Provide recent pest control reports from past 3 months if available
