Observation: Few fruit flies observed in the donut preparation area and in the dishwashing area. The manager mentioned that pest control completes inspections and did not note this issue.
Corrective Action(s): Install pest fly traps (yellow tape) in the back area to rid current fruit fly population. Ensure garbages are taken out more frequently to prevent fruit fly harbourage/breeding. Monitor fruit fly population and if population increases, contact pest control company to assess the situation.

1. Temperatures:
- Walk-in cooler at 4'C
- Walk-in freezer at -18'C
- Under-counter cooler #1 at front at 3'C
- Under-counter cooler #2 at front at 3'C
- Sandwich cooler at front at 3'C (top and bottom)
- Temperature logs maintained (digitally)

2. Sanitation:
- General sanitation of premises satisfactory
- All hand washing stations supplied with liquid soap, paper towel, hot and cold running water
- QUATS sanitizer pails and ice cap wand sanitizer measured at 200 ppm QUATS concentration
- High temperature dishwasher measured at 80.0'C
- 3-compartment sink available with drain plugs

3. Storage:
- All food stored at least 6 inches off the floor and covered
- Eggs stored on bottom shelf in the cooler
- Chemicals stored away from food
- Equipment and single-use disposable cups/utensils kept in sanitary manner

4. Pests
- Pest control company contracted to complete monthly inspections

5. Administration:
- Health permit visible on premises
