Observation: The low temperature dishwasher was not able to sanitize food contact surfaces adequately.
Corrective Action(s): All food contact surfaces cleaned/sanitized by the low temperature dishwasher requires re-cleaning and sanitizing by manual dishwashing method. Ensure all food contact surfaces (e.g., knives, pots, etc.) are cleaned and sanitized using manual 2 compartment sinks method. Correct this by: December 22, 2021.

Observation (CORRECTED DURING INSPECTION): Single use hand towels were not available at the hand washing station. The hand towels were replenished at this time.

Observation: Observed moderate amount of fruit flies near the hand washing sink.

Observation (CORRECTED DURING INSPECTION): Observed moderate amount of fruit flies activity by the hand washing sink. Fruit flies were appeared to be attracted by staff's lunch that was stored outside (banana, peeled red onion, etc.). Manager removed the attractants at this time. Did not observe fruit flies issue elsewhere in the kitchen.

Observation: Low temperature dishwasher: chlorine sanitizer residue measured at <50 ppm at the plate level. Attempted to prime the tubes. Ran multiple cycles. Confirmed the source bucket is not empty. Operator cited that the source bucket was changed recently (~1 month ago).
Corrective Action(s): Low temperature dishwasher is to be serviced immediately. Ensure minimum of 50 ppm of chlorine is detected at the plate level after the sanitizing cycle. Correct this by: December 22, 2021.

Routine inspection conducted. The facility was not in operation at the time of inspection:
General sanitation is adequate.
All coolers measured at 4C or below.
All freezers measured at -18C or below.
Kitchen hood last cleaned 2020 June 24. Kitchen hood is to be cleaned professionally as soon as available.
Reviewed manual dishwashing method with the operator.
