Observation: High temperature sanitizing dishwasher max temp 67 C

Fry cutter heavily soiled with food debris.

Wiping rag sanitizer containers approximately 100 ppm Quaternary Ammonia.
Corrective Action(s): High temperature dishwasher is to be repaired / adjusted to achieve a minimum of 71 C to adequately sanitize dishware.

Fry cutter is to be cleaned and sanitized. All food equipment, food utensils and food contact surfaces must be cleaned of all debris, then sanitized after use. Food equipment, utensils and work surfaces in constant use for the same purpose must be cleaned and sanitized a minimum of every 4 hours or when changing uses.

Refresh sanitizer solution for wiping rag containers to 200 ppm Quaternary Ammonia. Sanitizer should be replaced every 2 hours to maintain concentration.

Observation: Hallway space at staff washroom entrance - lower portion of wall has holes that can allow for movement and harborage of rodents.
Corrective Action(s): Seal off holes with metal flashing or equivalent pest proof materials.

Observation: Cleaning required in the following locations:
- walk-in cooler, walk-in freezer and kitchen entrance doors - hand contact surfaces have debris build-up
- mouse droppings observed on floor corner adjacent to walk-in cooler, floor under kitchen double boilers, on wheeled cart by sushi prep area, inside cabinet below sink adjacent to ice cream machine and around adjacent door.
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.

Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine 1 tsp bleach / 1 liter water) after cleaning.
