Observation (CORRECTED DURING INSPECTION): Pizza cutting knives observed with food debris affixed and jammed inbetween the tables.
Corrective Action(s): Knives must never be stored jammed inbetween tables as this is a possible source for cross contamination.
Once knives have been used they are to be removed and placed in the warewashuing area to be cleaned and sanitized.
Clean knives are to be stored in a separate area ie magnetic strip or container.

Observation: Premises observed to have a mouse infestation as droppings observed through out the premises
Corrective Action(s): Premises has a pest control contract.
Until pest problem is eliminated the pest control company needs to come by more frequently to identify and eliminate all possible points of entry.
Until pest control infestation has been eliminated, bin all food items that can potentially be chewed into ie flour/sugar bags. Use containers with tight fitting lids
Carefully clean and remove all observed old droppings in order to identify where mice may be still getiing access. Sanitize all areas where droppings have been removed.
Store all equipment on higher shelving.

Observation: Several points of entry identified that were not identified by the pest control company
Corrective Action(s): Seal entry point at front door, back door near the office, botttom of door to exterior electrical room and seal observed wall entry points observed in the wall below the dishwasher.
Remove old rags and clutter observed in store rooms, under stairs storage areas that can be used as harbourage points. Ensure base of doors below these rooms have had door sweeps installed in order to contain the pests and place traps in these rooms.

Observation: Top exterior side of the pizza prep cooler observed with spiiled food debris.
Floor inside the walk in cooler observed with spilled food debris
Corrective Action(s): Clean and remove spilled food debris.
Premises must do a thorough cleaning at the end of every day to remove all spilled food items until the pest problem has been eliminated.
