Observation (CORRECTED DURING INSPECTION): Chemical dishwasher measured at 0 ppm chlorine concentration with test strip during inspection. Staff contacted dishwasher company to repair on site during inspection. Dishwasher chemical concentration later measured at 100 ppm chlorine concentration with test strip.
Corrective Action(s): Check chemical dishwasher sanitizer concentration at least once per day (or as per approved sanitation plan) and maintain a record on site to be verified by the health inspector. Ensure sanitizer concentration is at 50-100 ppm chlorine concentration with test strip to adequately sanitize all dishes, utensils and equipment.

Observation: Few house flies observed on site during inspection.
Corrective Action(s): Place yellow sticky fly traps in non-food preparation areas to remove current house fly population. Keep all windows and doors closed as must as possible to prevent fly entry on site. Keep all food covered to prevent potential contamination.

Observation (CORRECTED DURING INSPECTION): Dishwasher is not able to dispense chemical sanitizer during the inspection. Dishwasher repair company was contacted during inspection. Dishwasher was repaired on site.
Corrective Action(s): Ensure dishwasher is able to dispense at least 50 ppm chlorine concentration by checking chlorine concentration at least once daily. If the dishwasher is not functioning, contact repair company immediately to repair and to ensure dishes are being sanitized adequately.

- Temperature logs are maintained - good!
- General sanitation of premise is satisfactory
- Hand washing stations are supplied with hot and cold running water, liquid soap and paper towel
- QUATs sanitizer in spray bottles (X3) measured at 200 ppm QUATs concentration with test strip
*REMINDER: ensure chemicals are stored in accurately labelled spray bottles to prevent potential chemical contamination of surfaces and food
- Glasswasher measured at 100 ppm chlorine concentration with test strip
- 2-compartment sink available on site
- Ice machine observed in general sanitary condition
- Equipment is stored in a sanitary manner
- Food observed stored at least 6 inches off the floor and covered
