Observation (CORRECTED DURING INSPECTION): Food equipment and utensils (including deli slicer and several dishes in clean storage rack) observed to be visibly contaminated with spills and/or debris.
Corrective Action(s): Ensure all food equipment, utensils, and food contact surfaces are properly washed, rinsed, and sanitized to limit potential contamination of food. Date to be corrected by: Today.

302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
Observation: 1. No sanitizer (chlorine) was detected in the rinse residual at the utensil surface for the low temperature mechanical dishwasher. Note: Dishwasher was run for three cycles.
2. One spray bottle of bleach water (chlorine) sanitizer was observed in kitchen; however, spray nozzle was not functional. No other surface sanitizer available. *CORRECTED*
Corrective Action(s): 1. Ensure dishwasher is serviced so that it is in good working order and capable of properly sanitizing equipment, utensils, and food contact surfaces. Minimum required rinse residual is 50 ppm chlorine. All equipment, utensils, and food contact surfaces must be properly washed, rinsed, and sanitized to limit potential contamination of food. Date to be corrected by: July 12, 2022.
2. Two buckets of bleach water sanitizer were prepared at 100 to 200 ppm chlorine. Wiping cloths stored in buckets. Ensure surface sanitizer is always available at the correct concentration for effective sanitizing of food contact surfaces.

Observation (CORRECTED DURING INSPECTION): 1. Multiple uncovered food items observed in kitchen and dry storage area (dry noodles, spices, and other dry goods). In addition, multiple uncovered food items observed in cooler.
2. Six open cans of food (corn, condensed milk, coconut milk) observed in coolers.
Corrective Action(s): 1. Food items that were uncovered and may potentially have been contaminated due to pest infestation were discarded at the time of inspection. This includes one large open bag of flour, and multiple smaller open containers and open bags of salt, sugar, and spices. Items stored in coolers were covered. Ensure all food is properly covered and sealed in order to protect from potential contamination.
2. Discarded at time of inspection. Ensure that once canned food products are opened, they are immediately transferred to appropriate food grade containers with lids. Do not store in the original can as once the can is opened, it may potentially corrode and contaminate the food.

Observation (CORRECTED DURING INSPECTION): Five sealed bags of sliced raw beef observed to be thawing at ambient temperature on storage rack. Beef was still partially frozen.
Corrective Action(s): Beef placed into cooler at 4 degrees C or less. Ensure potentially hazardous foods such as raw meats are properly thawed in order to limit the growth of potential pathogens.

Observation: Multiple living and dead cockroaches observed throughout the premises.
Corrective Action(s): Closure order issued. Have the premises serviced by a licensed pest control operator in order to limit the pest infestation. Ensure all areas of the premises are thoroughly cleaned and sanitized to remove food and other debris. Date to be corrected by: July 12, 2022.
