A large portion of the wall behind the dishwasher and the 2 compartment sink has been taken out 3. The screen door at the back door was not tightly fitting and had gaps at the bottom Corrective Action(s): 1. The owner shall contact the pest control company immediately and get the facility inspected and set up traps as necessary. The owner is to send a copy of the pest control inspection report to the health inspector via email. 2. The wall behind the sink shall be covered and sealed properly. Thorough cleaning was completed in the hard-to-reach areas. Shelves were free of food debris and grease. Dishes and utensils were kept in a sanitary manner. Pest control issue still remains outstanding. Follow up inspection will be conducted in 2 weeks to confirm the structural repairs.
