Observation: Lid of dry storage bin for dry rice was not intact.
Corrective Action(s): Operator has placed a second lid, in addition to the previous lid, over this dry storage bin to cover the rice an interim measure. Repair or replace the lid of the dry storage bin to fully enclose it over the dry storage bin; Correct within 1 week.

Follow up inspection:
-Previously affected areas had been cleaned and sanitized to remove rodent droppings.
-Plates appeared clean in the storage area.
-Empty pots were covered in the dry storage room.
-Empty soup bowls on a storage shelving unit were stored in pest proof containers with tight fitting lids.
-Pest control program is in place on a monthly basis. Continue with regular monitoring, cleaning, and pest control measures.
-Paper towel dispenser was re-installed in the women's washroom.
-Men's washroom was accessible and handsink was properly supplied.
